The Cause Road Map© makes your investigation task easier because it covers both human performance and hardware failure causes / modes. Constructed as a taxonomy, the graphical structure separates the various cause factors into groups with common characteristics and sub-divides these groups into a hierarchic order based on their relative significance or importance.
